This page describes how the Morrow Creek Dentistry concept site handles the information you send through its request form. Morrow Creek Dentistry is a fictional practice created by Locafy for portfolio purposes. No patients, no appointments and no treatment exist behind this site, and no patient data is collected or processed by a dental practice through it.

What the form collects

The request form asks for your name, your phone number, a choice describing what is going on, and a choice describing when you would like to be seen. It does not ask for your date of birth, insurance details, health history or payment information. Please do not type any of those into the form.

What happens to it

• The form sends one email message to the site owner at Locafy so the form can be tested.
• The site does not store form entries in its database, does not add you to a mailing list and does not share the message with advertisers.
• Spam protection uses a hidden field and a security token. It does not use third-party tracking.

Cookies and analytics

The site sets a small value in your browser session storage so the opening animation plays once per visit. It does not use advertising cookies. If analytics are added later, this page will say which.

Medical emergencies

This is a concept site. Do not use it to ask for real dental or medical help. For a real emergency call 911 or your local emergency number.
